I confirm Supervision will no longer be acting as our tax agent and Supervision will remove ourselves or any associated entities that we currently engage Supervision off their tax agents list. If my new provider does not add myself or associated entities to their list we may not get important information from the ATO which may result in penalties for non-compliance. I confirm Supervision will no longer be the registered address for any companies I am associated with (if applicable). I will arrange for a new address to replace the current address for this entity. I confirm recurring monthly fees (if applicable) will cease from the receipt date of the ethical letter, no fee refunds are given on fees already paid. I understand Supervision does not provide any accounting software data files. Supervision will provide email PDF reports to your new provider as soon as possible. I understand that paying 12 X Monthly fees (if applicable) in a financial year entitles me to Supervision completing a tax return for that financial year. If I have paid less than 12X Monthly fees, I need to pay the remaining monthly fees to be entitled to Supervision lodging my tax return.
